Normal Development For The Young Adult
18-20 Years Old
Oh my! Look at how much you have grown! Your 18-20 year old is starting to think about their purpose in life and has greater concern for others. They may want to become involved in social issues (community service) and is becoming more self-reliant. |

Transitioning To A New Provider
Although we hate to see you go, it is at this age that we need to start thinking about selecting a primary care provider who specializes in adults and can continue the healthcare services you will need in future years. This is not to say that we want you to leave - you can always stop in and say "Hi" and grab a lollipop!
Pediatricians typically see their patients up until age 18-20. Yet, there is not one correct age for kids to transition to an adult family provider - it all depends on the needs and desires of the patient. We encourage you to discuss this transition with your pediatrician to see if a change is right for you.
What about my medical records?
If you change providers, we will send a copy of your medical records to the new provider of your choice. This process is stared with the Release of Information form. You can use our form, or your new provider's office may provide a form for you. Either way, the Release of Information (ROI) is used to request records be sent to another party or to obtain records from another party. You would use this same ROI form to request a copy of your records for yourself. We can deliver your records in a variety of ways, but the most popular is to have us secure message them to you through the MyChart patient portal.
What happens to my MyChart Patient Portal Account?
Nothing! Your MyChart account will continue to be available to you 24/7. If you change your email address, just let us know and we will correct everything on our end. If you visit one of our Mary Bridge Children's Urgent Care clinics, your visit information will automatically be added to your portal account.
