Football, volleyball, soccer! If your child is looking to play sports in school, then most likely they will need a sports clearance form completed. The best time to have a sports clearance/physical is during your child's annual well child visit. Just let our staff know during scheduling and we will take care of the rest. Get started early and download our "Pre-Participation/Physical History Form" and bring the completed form with you to your visit.
Woodcreek Pediatrics - Mary Bridge Children's sports clearance forms are valid for one (1) year. Some schools, on the other hand, may accept the forms for a different period (some are 2 years) - please check with your school for the specifics. Woodcreek Pediatrics - Mary Bridge Children's will not sign a sports clearance form unless we have performed a well-child exam that year - so it is recommended you schedule an appointment with your primary care provider. We will gladly reprint a copy of your previous sports clearance, but if your school requires a new signature, then an appointment will be necessary. And also, please remember to have all your forms ready to go for your provider to sign during the visit.
Woodcreek Pediatrics - Mary Bridge Children's has coordinated with many of the school districts to ensure our electronically signed documents are accepted in place of the sports clearance forms provided by your school. Many times we can simply attach the school's form with the Woodcreek Pediatrics - Mary Bridge Children's generated printout. Sometimes, this is not enough and you may be asked by your school to have the form filled out differently - usually the school district is looking for the provider's physical signature (our forms are electronically signed since we keep all of our medical records electronically). This problem is easily solved. Just stop by one of our offices and we can stamp the document - no appointment necessary.
My Pediatrician Doesn't Have An Opening, What Can I Do?
Sports physicals can be provided by the Mary Bridge Urgent Care clinics that we operate; however, please be aware that insurance may not cover sports physicals delivered in an urgent care setting. Effective Aug 1, 2024, the price for this service is $250 (subject to change) except for uninsured patients and those covered by Apple Health, who are charged $100 due at the time of service.
Baseline Concussion Screenings
The Puyallup School District and a few others require a baseline concussion screening prior to a student athlete participating in select sports. Sadly, Woodcreek Pediatrics is no longer able to provide this service for you. We recommend contacting your school for a list of resources or booking your appointment with Olympic Sports & Spine.
