Using MyChart to Access Medical Records
MyChart is MultiCare's secure, web based medical information portal. You can login through any computer, tablet or mobile device to securely view the clinical records for you and your children. Login to MyChart to view the following information:
MyChart Accounts for Family Members or Other Representatives
Parents are given full access to their child's MyChart account through age 12. Once their child turns 13, parental access is limited.
How to obtain a copy of your medical records
If you are just looking for a copy of your recent visit, give our office a call or send us a secure message through the MyChart patient portal and most of the time we can simply retrieve the document for you.
If you are looking for a complete chart copy, copies of your records to be sent to a third party or other more complex requests, please complete the Release of Information Form available on our Forms page. Please pay special attention to the instructions on this form as it must be filled our completely for accurate processing. If the records pertain to a minor child and you are requesting content that contains mental health, reproduction, substance abuse or other sensitive material, the minor child will be required to sign the form if they are age 13 or older (or any age for reproductive health records). For assistance in completing a Release of Information form, please ask one of our front desk staff.

We process medical records request as quickly as possible, but please allow up to 30 business days for your request to be processed. If you have questions about the status of your request, please call 253-848-8797 and ask to speak with our medical records department.
How to change your medical record
If you believe that we have captured incorrect information in your medical record, you have the right to request that we amend it. To do so, please fill out the Request for Amendment Form on the MultiCare website and submit it as specified on the form.
