Patient Registration Form

This form is required to register new patients and is used to update information on established patients.

Authorization for Health Care of a Minor

This form is needed for parents or legal guardians to authorize specific other individuals to bring a child in for care, and to make specific, limited healthcare decisions for his or her benefit, for a limited period of time. This form will be kept in the child's medical record.
